To avoid delays in processing please submit applications by email only.

To facilitate same day processing and issue of a licence and/or registration, please ensure notifications (relating to settlement of purchase, completion of premises and commencement of operation) are submitted by email no later than 2.00 pm.

This section provides access to a series of application and notification forms relating to licensing, registration of premises and other approvals.

Please select the relevant form, download, complete and make the appropriate payment. Email the completed form & payment receipt together to applications@pharmacy.vic.gov.au (The completed form will not be processed without providing the payment receipt.) Application fee payment can be made by clicking on the relevant application form.
