To facilitate same day processing and issue of a licence and/or registration, please ensure notifications (relating to settlement of purchase, completion of premises and commencement of operation) are submitted by email no later than 2.00 pm.

Email the completed form & payment receipt together to applications@pharmacy.vic.gov.au (The completed form will not be processed without providing the payment receipt.) Application fee payment can be made by clicking on the relevant application form.
